Almost everyone has a pocket. The fullness under the chin that appears in every photograph taken from below. The soft roll at the bra line. The lower tummy that stays after months of discipline have worked everywhere else. In Accra we hear the same story weekly: the weight is fine, the gym is regular, and one specific area simply refuses to follow.
Fat dissolving injections exist for exactly that pocket. They are one of the most requested body treatments at our clinic and one of the most misunderstood — sold in some places as a slimming shortcut, dismissed in others as a gimmick. The truth sits between, and depends on where the fat is and what the skin above it will do once the fat has gone.
How injectable lipolysis works
The injectable is based on deoxycholic acid, a substance your own body produces to break down dietary fat in the gut. Injected in a measured grid directly into a fat pocket, it breaks open the outer wall of the fat cells it reaches. Those cells break apart, and over the following weeks your lymphatic system and liver clear the released contents in the same way they clear fat from any other source.
Two consequences follow from that mechanism. The first is that the treatment is permanent in a specific sense: a fat cell that has been destroyed does not come back. The second is that it is local. It affects only the cells the solution reaches, which is why the injections are mapped so precisely, and why the treatment is useless for anything larger than a pocket. Put plainly, fat dissolving is a contouring treatment for stubborn localised fat, not a weight-loss treatment. It serves best people already near a weight they are comfortable at, whose remaining frustration is one or two areas that will not shift.
The areas that respond
Under the chin and along the jowls
This is where the treatment made its name, and it remains the most reliable area. The fat under the chin is usually a discrete pad with defined edges, and a sharper angle between chin and neck changes a profile more than almost anything else. We treat it under facial fat dissolving, and it is one of the few treatments that men and women request in nearly equal numbers. Jowls respond too, though more gradually, and precision matters there because a nerve controlling the smile runs close to the jaw.
Arms, bra line and back
The soft fold at the bra line, front and back, is often a small, well-defined pocket that suits the treatment well and rarely has much loose skin over it — a common request from clients in Accra who want a cleaner line under fitted clothes. The inner upper arm often carries a pocket that responds too. The question there is the skin: if the arm waves back mainly because of laxity rather than fat, dissolving the fat can make the looseness more obvious, and arm tightening or a combined plan is the better route.
Lower abdomen, flanks, inner thighs and knees
The small lower-belly pouch and the pockets above the hips respond, provided they are genuinely fat rather than a mix of fat, separated abdominal muscles and loose skin — post-pregnancy tummies in particular need honest assessment first. The inner thigh pocket that rubs, and the fatty pad on the inside of the knee, are small and well suited. The outer thigh is larger and more spread out, and results there are modest.
The areas that do not
Large, spread-out fat — a whole abdomen, the full outer thigh, generalised fullness — is beyond what a grid of injections can treat: the volume of solution needed would be unsafe and the result still uneven. Fat that sits deep behind the abdominal muscle, giving a firm, rounded belly rather than a soft pinchable layer, cannot be reached at all; that is visceral fat, and it responds to diet and exercise, not injections. We can tell you which kind you have by pinching it.
Cellulite is not a fat-pocket problem either. It is structural — fibrous bands tethering the skin over the fat — and dissolving fat beneath dimpled skin does not release the bands. Where dimpling is the real concern, cellulite reduction protocols work on the structure instead, and the two treatments are sometimes combined on the thighs.
Sessions and spacing
The area is mapped, numbed and injected in a grid — around 20 to 30 minutes. Most areas need two to four sessions, spaced about six weeks apart. That spacing is not arbitrary: the body needs four to six weeks to clear each round of released fat, and treating sooner adds swelling without adding result. The number of sessions depends on the size and density of the pocket, reviewed at each visit rather than fixed in advance.
It looks worse before it looks better
This deserves its own section because it is the moment clients ring us. Within hours of treatment the area swells, and for several days it can look noticeably fuller than before you started — under the chin especially, people describe acquiring a second chin they did not have that morning. Tenderness, warmth, firmness and sometimes bruising accompany it, and small nodules can be felt under the skin as the fat breaks down.
All of it is the treatment working: the swelling is inflammation, and inflammation is the clearance process. It settles over a week or two, the nodules soften over several weeks, and the actual result — the flatter contour — becomes visible at around four to six weeks, so we do not judge a session before then. Plan the first session away from weddings, photographs and, if you are travelling to Ghana for treatment, the return flight.
Who should not have it
Fat dissolving waits during pregnancy and breastfeeding. It is not placed over active infection or inflamed skin, and it is not appropriate for anyone with significant liver or kidney disease, since those organs clear the released fat. People on blood thinners need medical clearance. It is also the wrong treatment for anyone hoping to change a whole body rather than a pocket, and for skin that is already very loose over the pocket, for the reason below.
On darker skin tones the news is good: the solution works beneath the skin surface and carries no pigmentation risk of the kind that makes lasers and peels demand expertise. Bruising can leave temporary marks, and these fade.
The skin afterwards
Fat gives skin something to sit on. Remove a pocket and the skin above it has to contract to follow, which young, thick skin does well and older, thinner, sun-exposed or stretched skin does less well. This is the most important question in the assessment, and why we look at the skin as hard as the fat.
Where laxity is likely, we plan skin tightening alongside or after the dissolving sessions, so collagen is being built in the skin while the fat beneath is cleared. Under the chin, fat reduction is often completed with jawline enhancement, where filler along the jawbone gives the newly revealed line something to sit against — a sequence popular with men who want the profile finished rather than merely reduced.
Honest expectations by area
Under the chin: a visibly cleaner angle in most people after two to three sessions, and the best value of any area. Jowls: a softening of heaviness rather than a sharp new line. Bra fat: a flatter line under fitted clothes. Arms: real slimming of a pocket, with the caveat that the skin decides how it reads. Lower abdomen: a flatter pouch, provided muscle separation and loose skin are not doing most of the work. Inner thighs and knees: reliable, because the pockets are small.
Nowhere does it deliver a different body. Everywhere it works, it delivers a version of your body with one annoyance removed, which is what most people actually wanted.
Does it hurt?
The area is numbed, and the injections themselves are quick. The stinging and burning for a few minutes afterwards is the part people remember, followed by tenderness for a few days. Most describe it as uncomfortable rather than painful.
Is it really permanent?
The treated fat cells are gone. The remaining cells can still enlarge if you gain weight, so the result holds best on a stable weight — and significant gain can make the area look fuller again, though usually less than it would have.
Can I have it at the same time as other body treatments?

When will I see the result?
Around four to six weeks after each session, with the full result of a course visible about six weeks after the last one. Photographs in the same light at each visit are the honest measure; the mirror the morning after a session is not.
